SGGPCP is committed to an integrated health and community service system which places the consumer at the centre of service delivery.  Encouraging agencies in our catchment to work together promotes a coordinated and streamlined journey for those with chronic and complex care needs.

SGGPCP aims to:

  • Support agencies in using a standard approach to service coordination including initial contact, initial needs identification, assessment and care planning
  • Support a regional approach to service coordination with other partners.
  • Encourage partnerships between health and community organisations for better-integrated service coordination across the catchment
  • Support further development of e-health opportunities including e-referral and Telehealth
